We are no longer requiring patients to complete our COVID-19 Pre-Appointment form, however please let our staff know if you have:

  • Had a fever or felt ill within the last 10 days

  • Been in contact with a known COVID patients within the last 10 days

  • Traveled outside of the U.S. within the last 10 days

What are we asking our patients to do?

  • We recommend that enter the office with a mask. It is no longer a requirement as of March 2022.

  • We recommend that you use the hand sanitizer provided upon entry to our lobby.

  • Please be respectful of others’ decision to wear or go without a mask.

What are we doing to combat the spread of COVID-19?

  • All staff members who provide care will be required to wear an ASTM level 3 mask.

  • Staff will continue to practice proper hand hygiene, as we always have before. This will include frequent washing and sanitizing.

  • Each room, as always, will be completely disinfected with hospital-grade surface disinfectants. This will include all handles, doorknobs, chairs and other frequently touched surfaces and will happen, office-wide at least hourly to keep up with traffic.

  • Our treatment rooms have all had their ventilation altered to constantly exchange air.

  • Air purifiers have been installed in each treatment room and the lobby.
